If you are interested in enrolling in the Guardian Scholars Program, contact the program advisor at vqgarcia@ucdavis.edu or (530) 752-1211 and submit the items listed below.

Enrollment Documents:

• Guardian Scholars Program Enrollment Form

• 1-2 page typed personal statement

Use the personal statement as an opportunity to introduce yourself. You may include information about your experience in foster care and how it has influenced your desire to attend college; your academic, personal and career goals; someone who has influenced you or other information that you feel is important for us to know.

• Copy of the ward of court documentation*

If documentation cannot be provided, contact the Guardian Scholars Program Advisor at vqgarcia@ucdavis.edu or (530) 752-1211

• Student Agreement which indicates your acceptance of the terms outlined
